DERMOGENESIS WITH MESENCHYMAL STEM CELLS: A FABULOUS REMEDY FOR POST ACNE
DEPRESSED SPOTS
Post acne depressed spots are due to localized fat atrophy and disruption of dermis. Treatment is very hard because of many
reasons as difficulty is excision due to their small size and large number. Subcision followed by filling is not adequately
treating the deformity. Despite of multi modalities, outcome is not satisfactory. This study compared long-term effects of
conventional fat grafting and ex-vivo expanded Mesenchymal Stem Cells (MSCs) enriched lipofilling for the treatment of post
acne depressed spots.
After informed consent, 40 patients having post acne spots on face were recruited and after explanation of types of treatment,
placed in two groups depending upon their choice. In lipofilling only group (LF-G), micro fat was harvested and filled intra
dermally and sub dermally in the depressed post acne spots. In MSC enriched lipofilling group (MSC-LF), fat was harvested,
MSCs were isolated, cultured to enrich fat before lipofilling intradermally and sub dermally in depressed spots. Improvement
was noted after one year. Increase in dermal thickness was confirmed on histological sections of small punch biopsies.
Mean age of patients was 26.32(±3.34) and 72% patients were females. Cheeks-only were involved in 57% cases. In MSC-LF
group, improvement in dermal thickness was 1.2(±0.6)mm in comparison to LF-G group which was 0.5(±0.3)mm. Patient and
physician satisfaction was more in MSC-LF group.
MSC enrichment is having pronounced effect in addressing post acne depressed spots by filling as well as increasing dermal
thickness.

IL-13 IN ATOPIC DERMATITIS: UNLOCKING MECHANISMS OF NEW BIOLOGIC THERAPIES
Atopic dermatitis (AD) is a chronic inflammatory skin condition characterized by immune dysregulation, with IL-13 being a key
cytokine involved in its pathogenesis. Elevated IL-13 levels are observed in both skin and blood of AD patients, correlating
with disease severity and chronicity across different age groups and ethnicities. Dupilumab, an earlier biologic treatment,
targets the interleukin (IL)-4 receptor alpha subunit, inhibiting both IL-4 and IL-13 signaling. More recent biologics, such as
tralokinumab and lebrikizumab, specifically target IL-13, offering a more focused approach to modulating this cytokine. These
IL-13 inhibitors have shown efficacy in reducing inflammation and improving skin barrier function. In summary, IL-13 plays a
significant role in AD, and biologics targeting IL-13, including tralokinumab and lebrikizumab, provide alternative treatment
options with the potential to improve disease outcomes.

BEYOND SUNSCREEN: LAYERING PROTECTION AGAINST KEY SKIN EXPOSOME FACTORS
Pigmentation concerns, including melasma and post-inflammatory hyperpigmentation are amongst the most prevalent
dermatological challenges in Asia. Emerging research highlights the critical role of the skin exposome—including ultraviolet
(UV) radiation, pollution, infrared radiation, blue light, and oxidative stress—in the onset and exacerbation of
hyperpigmentation. While broad-spectrum sunscreens are essential in pigment management, evidence suggests that a
multi-layered defence strategy—combining advanced UV filters with antioxidants, DNA repair enzymes, and
pigment-regulating agents—can provide superior protection and correction.
This presentation will examine the interplay between environmental aggressors and melanogenesis, emphasising the role of
potent antioxidants, ultra broad-spectrum filters and iron oxide-based formulations for blue light protection, and emerging
skincare technologies in mitigating pigmentary disorders. Additionally, we will explore the latest clinical data on combining
photoprotection with depigmenting agents, optimising skincare protocols for patients prone to hyperpigmentation.

PICOSECOND LASER-INDUCED OPTICAL BREAKDOWN: A NOVEL APPROACH FOR REVERSING
PHOTO-AGING AT THE MOLECULAR LEVEL
Background: Photoaging, driven by prolonged ultraviolet (UV) exposure, significantly
impacts skin aesthetics and health. Traditional treatments vary in efficacy and
invasiveness. This study evaluates the effectiveness of 755 nm picosecond laser-
induced optical breakdown (LIOB) combined with a diffractive lens array as a novel,
minimally invasive treatment for photoaging.
Methods: A controlled study was conducted on female BALB/cAnN.Cg-
Foxn1nu/CrlNarl mice exposed to UVA and UVB radiation to induce photoaging. Mice
were treated with the PICOSURE® 755 nm picosecond laser. Changes in wrinkle
formation, skin elasticity, collagen degradation, and skin barrier function were
assessed. Key proteins analyzed included Smad 2/3, p-p38, and MMP-9.
Results: Laser treatment significantly improved skin texture, reducing wrinkle formation
and erythema. Collagen intensity increased significantly post-treatment, with marked
reductions in MMP-9 expression (47.3% decrease, p

MANAGEMENT OF BLEPHAROPTOSIS
Eyelid ptosis is not only a cosmetic concern but also a visual disturbance. To elevate the eyelids, the protection of cornea has
to be considered. Significant lagophthalmos has be avoided in pursuit of a "big eye". Therefore, preoperative evaluation is
mandatory, including margin reflex distance (MRD), levator muscle function, Bell's phenomenon, tear film stability and tear
production (e.g. Schirmer test). Levator advancement or resection is only reserved for patients with small MRD. Those with
redundant upper eyelid skin such as dermatochalasis usually do not need manipulation of levator muscle. Patients with brow
ptosis can be suggested to have a brow lift. An incision close to the brow usually leaves a significant scar. However
endoscopic brow lift is technique and cost demanding. Patients with poor levator muscle function frequently end up with
severe lagophthalmos and cornea epithelial defects pain or ulcer in an aggressive resection of levator muscles, which is
strongly not suggested. In conclusion, a harmless ptosis surgery is the best policy in upper eyelid blepharoplasty.

INSULIN RESISTANCE IN AGING PROCESS. HOW TO CONTROL?
Insulin resistance is a condition in which the body's cells do not effectively respond to insulin, the hormone produced
by the pancreas that allows glucose (sugar) to enter the cells through what are known as sugar channels. This
condition is a major factor in the development of type 2 diabetes and pre-diabetes.
Pre-diabetes is defined as when blood sugar levels are between 1.05 g/L and 1.25 g/L, confirmed by repeated testing
one month apart. If blood sugar levels exceed 1.25 g/L, it indicates type 2 diabetes.
Controlling insulin resistance is crucial because it helps prevent the progression to high-risk conditions, particularly
cardiovascular diseases and inflammatory diseases caused by excess sugar in the blood. Therefore, managing insulin
resistance should be a priority in preventive medicine and aging management.

The deviated nose presents a tremendous challenge for rhinoplasty surgeons, especially the correction of the cartilaginous
dorsum deviation. In this study, we introduce the surgical techniques of correcting a deviated cartilaginous vault by creating a
new center of the lower third using a caudal septal extension graft combined with unilaterally extended spreader grafts.

EXPLORING UV-RELATED SKIN CANCER IN SYSTEMIC DISEASES WITH SPECIAL FOCUS ON DIABETES
Skin cancer, notably induced by ultraviolet (UV) radiation, represents a significant concern among patients with various
systemic diseases, including organ transplant recipients, chronic kidney disease, and autoimmune conditions such as
rheumatoid arthritis and psoriasis, all of which have increased skin cancer risks. Interestingly, diabetes mellitus demonstrates
an unusual pattern in susceptibility to photocarcinogenesis. Although chronic hyperglycemia typically increases cancer risks in
solid organs, the association with non-melanoma skin cancer (NMSC) remains unclear, with conflicting findings and
unexpectedly lower incidences reported in some studies.
This presentation reviews current evidence on UV-related skin cancer risks associated with systemic diseases, emphasizing
diabetes and its unique effects on epidermal responses to UV radiation. We highlight recent key findings from our
investigation, revealing that elevated glucose concentrations significantly modulate human epidermal keratinocyte responses
to UVB radiation, potentially explaining diabetic skin's altered resistance to UV-induced carcinogenesis. These findings
deepen our understanding of diabetic skin biology and suggest novel pathways for developing targeted strategies to prevent
skin cancer.

CONFESSIONS OF A FACIAL OVERFILLED SYNDROME VICTIM - HOW I MAKE THEM RETURN TO NATURE
A 40-year-old female came to our clinic and complained that she had received 1000 shots of Ultraformed 3 in other clinic to
improve her sagging, and the main treated areas over the middle, lower face and neck. However, on the third day, she began
to experience abnormal swelling, pain and discomfort over a large area of ??her face. During the consultation, it was found
that there was no obvious superficial redness but evident symmetrical swelling. The patient recalled that she had received
PDLLA injection 3 years before receiving the HIFU treatment. Ultrasound examination revealed that the previuos injection
sites such as temples, infraorbital hollws, lateral cheeks, NLF, etc. had very obvious large-scale hypo-echoic mass and tissue
edema reaction. After about 2 months of oral doxycycline and prednisolone treatment, the swelling was significantly improved,
but there was still visible and palpable localized bump in the aforementioned areas. After 2 months, I performed intralesional
triamcilone under ultrasound guidance and the condition finally returned to normal. I would also like to share my personal
treatment alogorism and experience for filler complications, hoping to give everyone a basic treatment idea when dealing with
filler complications.

Chronic urticaria is typically characterized by the presence of wheals that spontaneously resolve within 24 hours. However,
when wheals persist for an extended period, the diagnosis of urticarial vasculitis (UV) should be considered, and a skin biopsy
should be performed. A diagnosis of UV is confirmed if histological evidence of vasculitis is present. In cases where vasculitis
is not histologically evident or a biopsy is not conducted, the condition may be classified as UV-like severe chronic
spontaneous urticaria (CSU). The prolonged nature of lesions and high symptom burden associated with UV and UV-like CSU
can significantly impair the quality of life of affected patients. Despite this, a consensus on treatment remains lacking. These
patients often require systemic corticosteroids or other immunomodulatory therapies. In this presentation, I will share my
experience with the use of Janus kinase (JAK) inhibitors in treating these conditions.

affected by various pathological conditions. Due to continuous exposure to physical trauma and other external stimuli, nail

Injectable collagen has been extensively utilized in anti-aging treatments due to its ability to enhance dermal structure,
promote skin elasticity, and stimulate collagen production. In this presentation, the mechanism of action of Heterologous Type
I Collagen (HTIC) will be explained, showcasing its transition from anti-aging applications to therapeutic uses in dermatology.
We explore the efficacy and safety of HTIC in treating Vulvar Lichen Sclerosus (VLS), a chronic inflammatory dermatosis that
significantly affects patients' quality of life and poses challenges due to the limitations of current treatments. Additionally, we
will discuss its potential role in genital rejuvenation.
Materials & Methods:
HTIC, in a dose of 100 mg dissolved in 4.5 ml of normal saline and 0.5 ml of lidocaine, was administered through intradermal
or subdermal injections in a grid pattern at 1 cm intervals to the affected areas. Patients received four treatments at two-week
intervals. The presentation will also include an evaluation of HTIC's effects on skin texture, elasticity, and patient satisfaction
in the context of genital rejuvenation.
Results:
Patients showed significant improvement in VLS symptoms, with a reduction in lesion size observed after the first treatment
and complete resolution by the third. Symptoms such as pruritus, soreness, and dyspareunia were notably relieved.
Additionally, patients reported enhanced skin texture and elasticity in the genital area, contributing to improved satisfaction
and well-being. No relapses occurred over a 12-month follow-up period, with only minimal maintenance required.
Conclusion:
HTIC emerges as a promising treatment for Vulvar Lichen Sclerosus, providing both therapeutic relief and aesthetic benefits
through genital rejuvenation. The positive outcomes support further research into HTIC as a valuable addition to treatment
options for VLS and beyond. The presentation will address these findings, along with the underlying mechanisms and broader
implications for regenerative medicine.

Abstract:
Introduction: Mesotherapy with non-cross-linked hyaluronic acid (HA) has long been a cornerstone in skin hydration
treatments, often enhanced by combining HA with biorevitalization cocktails containing vitamins, minerals, amino acids,
nucleotides, coenzymes, and antioxidants. A major advancement in the field of skin regeneration is the use of Type I soluble
collagen, which acts as a natural scaffold, promoting the adhesion, growth, and differentiation of connective tissue cells. Since
its emergence in 2015, Heterologous Type I Collagen (HTIC) has revolutionized dermal regeneration by significantly
enhancing fibroblast proliferation and collagen production. HTIC has been widely adopted in Cosmetic Medicine and
Anti-Aging Therapies, providing an optimal environment for restoring connective tissues and supporting physiological collagen
synthesis. For nearly three decades, lyophilized HTIC patches have been effectively used in Dermatology, Angiology, and
Vascular Surgery for treating skin ulcers, open wounds, scars, and bedsores. This study introduces HTIC, detailing its
mechanisms of action, therapeutic indications, and its safety and efficacy profiles.
Objective: This study aims to evaluate the biostimulatory effects of HTIC across various body regions, including the face,
neck, décolleté, hands, lips, periorbital area, arms, abdomen, and thighs/buttocks. Additionally, the study examines
patient-reported discomfort and potential side effects associated with the treatment.
Materials and Methods: A cohort of 423 women, with a median age of 42 years and exhibiting signs of cutaneous aging,
participated in this study. Each subject received intradermal injections of HTIC, with treatment areas including the face, neck,
décolleté, hands, lips, periorbital area (around the eyes), arms, abdomen, and thighs/buttocks. The treatment protocol
consisted of four sessions administered at two-week intervals. Outcomes were documented through pre- and post-treatment
photography, alongside clinical evaluations following each session. Pain levels during the injections were also recorded by
patients.
Results: Nearly all treated patients exhibited significant improvements in skin quality following HTIC therapy, including
enhanced elasticity and firmness in all treated areas. Patient-reported discomfort was minimal to moderate, with no adverse
side effects observed.
Conclusion: HTIC represents a state-of-the-art biostimulation treatment, offering sustained improvements in skin quality,
including enhanced elasticity and firmness across multiple body areas, including the face, neck, hands, lips, and periorbital
region. The treatment demonstrates a high level of efficacy and safety. Ongoing extensive clinical trials are underway to
further validate these findings. HTIC holds transformative potential for the future of dermal biostimulation therapies.

dermal biorevitalization by creating optimal conditions for the restoration of connective tissue and the physiological
neoformation of collagen.
Originally developed for anti-aging applications, HTIC has shown significant promise beyond its initial purpose, with potential
therapeutic benefits in treating a variety of dermatological conditions. This presentation aims to evaluate the efficacy of HTIC
not only as an anti-aging treatment but also as a dermal regenerator for conditions such as steroid-induced cutaneous
atrophy, atrophic acne scars, non-segmental vitiligo, plaque psoriasis, alopecia areata and androgenic alopecia. The
mechanism of action of HTIC and its anti-aging protocol will also be discussed.
Materials and Methods:
The study involved patients diagnosed with steroid-induced cutaneous atrophy, atrophic acne scars, non-segmental vitiligo,
plaque psoriasis, and alopecia areata. Each patient underwent four HTIC intradermal injection treatments, administered
biweekly. Photographic documentation was collected before and after each session, and clinical evaluations were conducted
to assess changes in skin condition. Patient-reported pain levels during the injection process were also recorded.
Results:
In the majority of cases, areas treated with HTIC showed significant improvement in skin quality, with many patients
experiencing complete resolution of their lesions. Reported pain during the injections was minimal to moderate, and no
adverse side effects were observed.
Conclusion:
HTIC proves to be an effective and safe dermal regenerator, offering substantial benefits not only in anti-aging but also in the
treatment of various dermatological conditions. Ongoing clinical trials aim to further validate HTIC as a comprehensive
treatment option, potentially revolutionizing the approach to conditions lacking definitive cures or universally effective
therapies
to achieving optimal outcomes.
The CDIS approach is proposed as a novel and comprehensive framework for acne scar revision, addressing Contour, Dents
(or Divots), Icepick scars and Skin quality as an innovative and holistic methodology.
Through illustrative case studies, the CDIS approach integrates scar-specific treatments with strategies to restore facial
contours and enhance skin quality, offering a holistic solution for acne scar revision. Contour correction restores facial
harmony using soft tissue injectables like hyaluronic acid and calcium hydroxyapatite, while dents/divots are treated with
subcision, energy-based devices and injectables to release tethered scars and stimulate collagen remodeling. Icepick scars
are managed with CROSS using TCA or phenol peels, surgical excision and laser resurfacing. Skin quality enhancement
involves interventions targeting dyschromia, vascular normalisation, hydration and sebum regulation, utilising topicals,
energy-based devices and injectables.
By combining scar-specific therapies with enhancements to skin quality and facial structure, the CDIS approach provides a
practical, patient-centred tool for clinicians and ensures superior outcomes for acne scar patients.

heterogeneity, posing diagnostic and therapeutic challenges. The advent of advanced genomic technologies, including
whole-exome sequencing (WES) and RNA sequencing (RNA-seq), has revolutionized the molecular diagnosis and treatment
landscape for these conditions. WES facilitates the identification of pathogenic variants in disease-associated genes, enabling
precise genetic diagnoses and the stratification of patients for targeted therapies. RNA-seq further complements these efforts
by uncovering transcriptional dysregulation, alternative splicing events, and gene expression profiles critical to understanding
disease mechanisms at the molecular level.
Gene therapy has emerged as a promising therapeutic modality for inherited skin disorders. In the context of EB, approaches

I begin with an overview of suture materials and needle types, followed by the selection of appropriate sutures for different
wound types. Emphasis will be placed on the edge eversion technique, a critical principle in achieving optimal wound healing
and minimizing scar depression. Key suturing techniques, including simple interrupted, running, vertical and horizontal
mattress, and subcuticular sutures, will be demonstrated with clinical insights on maximizing cosmetic and functional
outcomes.
In the advanced section, we will explore tension-relieving techniques, including buried sutures, pulleys, and alternative
methods such as ZipLine skin closure systems and the use of a urine catheter for horizontal suture approximation. These
approaches are particularly useful in high-tension wounds or anatomically challenging areas. Additionally, we will discuss
post-suturing scar minimization strategies, including the use of pulsed dye laser (PDL) and Fractional laser treatment after

Accurate differentiation between benign and malignant skin tumors is a critical skill in dermatologic practice. In this lecture, I
will share insights and practical approaches developed over years of clinical experience and dermatologic surgery. One of the
key themes of this talk is the impact of clinical experience on diagnostic accuracy. The ability to detect subtle clues—whether
visual, tactile, or intuitive—varies significantly depending on the level of experience. While textbook descriptions and
histopathologic criteria form the foundation, seasoned clinicians often rely on a refined "clinical instinct" developed through
repeated exposure to challenging cases. Through selected clinical cases, I will illustrate diagnostic challenges and
demonstrate how the integration of clinical impression, dermoscopy, biopsy strategy, and, in some cases, therapeutic
diagnosis can inform decision-making.
Particular focus will be given to: Role of dermoscopy in sBCC diagnosis and treatment monitoring, ddx of nevus vs pBCC,
early diagnosis of acral melanoma, actinic keratosis/SCC in situ, etc.

functional and anatomic assessment of cutaneous microcirculation improves the diagnosis and treatment for some skin and

This presentation will introduce two preclinical studies involving collagen stimulators conducted in porcine models:
Study 1 investigates the effect of different post-injection massage techniques on the distribution of poly-L-lactic acid (PLLA).
Through animal modeling and histological analysis, the study demonstrates that a single immediate massage is sufficient to
achieve uniform distribution, potentially reducing the inconvenience and adverse events associated with prolonged massage.

Background: BRAF V600E mutations drive melanoma progression and drug resistance. TGF-ß signaling contributes to this
process through TGFBR2 neddylation, preventing protein degradation. We investigated targeting TGFBR2 neddylation to
suppress metastasis and enhance BRAF inhibitor response. Methods: Gene Set Enrichment Analysis, site-directed
mutagenesis, MTT assays, migration and immunosuppression tests, and xenograft models were used to assess cellular and
tumor behaviors. Results: MLN4924 suppressed epithelial-mesenchymal transition and TGF-ß signaling. TGFBR2 knockdown
reduced migration and immunosuppression. Wild-type TGFBR2 restoration, but not neddylation-site mutants, recovered
cellular migration and drug resistance. Conclusion: Targeting TGFBR2 neddylation may combat melanoma.

EXOSOME IN REGENERATION AND CANCER
Exosomes enable communication between cells by transferring molecules. We found that stimulating the release of induced
exosomes (iExos) from stem cells triggers their transition to a non-stem cell state. When basal mammary epithelial stem cells
release iExos, they lose their stem cell traits. In contrast, when mammary luminal cells absorb these iExos, they acquire
basal-like stem cell properties and can generate mammary glands in vivo. This stemness transfer via iExos opens new
possibilities for regenerative medicine.
For example, the adult brain has limited ability to regenerate. As a result, injuries and neurodegenerative diseases often
cause lasting damage. Mesenchymal stem cells (MSCs), which can be isolated from adult tissues, have shown promise in
clinical trials. We discovered that MSC-derived exosomes triggered by an EP4 antagonist (GW-iExos) have significantly
improved therapeutic effects in treating central nervous system damage—including neuron loss, astrocyte hyperactivity,
inflammation, and blood-brain barrier disruption. In contrast, exosomes from untreated MSCs were far less effective against
such lesions, which are common in brain injury, stroke, Alzheimer's, and Parkinson's disease.
Similarly, breast cancer cells with a mesenchymal phenotype exhibit cancer stem cell (CSC) traits and resist chemotherapy.
By inducing iExo release, we can shift these cells from a mesenchymal/CSC state to a more epithelial, non-CSC state. This
process strips away CSC markers, mesenchymal markers, integrins, and drug efflux transporters, increasing the cells'
sensitivity to chemotherapy. This suggests that combining exosome inducers with standard treatment could effectively target
CSCs.
Finally, we found that tumor cells become more aggressive during progression by changing protein interactions that affect
how exosomal cargo—proteins and nucleic acids—is sorted. These discoveries have important implications for both
regenerative medicine and cancer therapy.

Radical surgery remains the cornerstone of melanoma treatment, with sentinel node biopsy serving as the standard method of
staging for patients with T1b tumors or greater. This staging technique provides critical information that informs the decision to
pursue adjuvant therapies, which may include checkpoint inhibitors (e.g., pembrolizumab, nivolumab) or targeted therapies
(e.g., BRAF and MEK inhibitors for BRAF-mutant melanoma). These therapies have been shown to significantly reduce the
risk of melanoma recurrence. Recent clinical trials of neoadjuvant immunotherapies, administered prior to surgery, have
demonstrated highly statistically significant improvements in event-free survival compared to standard adjuvant

Introduction:
Hair loss and hair depigmentation pose significant cosmetic and psychological concerns. Emerging evidence suggests that
through mechanisms involving cell proliferation, melanogenesis, and anti-inflammatory pathways.
Objective:
To evaluate the efficacy of exosome therapy in hair regrowth and repigmentation and explore its potential role in combination
with energy-based devices (EBDs) such as fractional lasers and microneedling.
Materials and Methods:
A review of existing studies and clinical applications of exosome therapy for hair loss and repigmentation, including AGA,
poliosis circumscripta, and alopecia areata, was conducted. The focus included RSCE application via noninvasive
electroporation, microneedling, and fractional picosecond laser therapy. Various treatment protocols, patient responses, and
assessment methodologies were examined.
Results:
Patients demonstrated significant hair regrowth, with increased follicular density and diameter. Dermoscopic evaluation
revealed enhanced melanogenesis, with previously white hair shafts displaying proximal black coloring. No severe adverse
effects were reported, with only transient erythema and mild discomfort observed.
Discussion:
The findings suggest that exosome therapy promotes hair follicle stem cell activation, enhances Wnt/ß-catenin signaling, and
modulates melanocyte function. Fractional laser and microneedling facilitate exosome penetration and augment regenerative
effects. The combination of regenerative medicine with EBDs presents a minimally invasive alternative to traditional
pharmacologic treatments.
Conclusion:
Exosome therapy, particularly when combined with EBDs, offers a promising treatment modality for hair regrowth and
repigmentation. Further large-scale studies are warranted to optimize protocols and elucidate long-term efficacy.

Seeking dermatological treatments might hide variable degrees of psychological discomfort which have been enormously
amplified after Covid pandemic. Recognizing Body Dysmorfic Disorder (BDD) is crucial, especially when practicing in
cosmetic and aesthetic medical-surgical environmentswhere patients seek procedures to address perceived physical
alterations. BDD is characterized by excessiveconcern over imagined of minor defects, leading to significant personal distress
and behavioral impairment. Practitioners should be able to recognize key indicators of BDD such as: excessive preoccupation
with appearance of specific anatomical regions; psychiatric comorbidities such as depression, anxiety, obsessive-compulsive
disorder (OCD) or social phobia; altered behavioral equilibrium such as excessive grooming, exaggerated make-up, skin
picking and doctor shopping; emotional recognition deficits such as: misinterpretation of facial expressions leading to
exacerbation of social rejection. BDD is more prevalent in younger, female, and single individuals with rates ranging from
2.9% to 57%. Structured Clinical Interview and implementation of specific BDD questionnaires may be of help. Early detection
of BDD is the main goal to avoid unnecessary procedures and improve patient outcomes with the help of specialized mental
health specialists. Refrain from performing inappropriate procedures on frankly or suspected BDD patients often leads to
dissatisfaction and can exacerbate potentially aggressive behavior. The use of a preliminary checklist to document the
assessment of BDD symptoms and well-structured informed consents can protect practitioners from potential legal issues.
Gentle referring patients affected by suspected BDD to mental health professional such as cognitive-behavioral specialist for
appropriate treatment is of fundamental importance.
Seeking aesthetic and cosmetic improvements is not a good strategy to "cure" underlying psychological uneasiness and might
even worsen the original situation should results do not match patients' clinical expectations. Dermatologists are called to
improve their screening capabilities in order to recognize potentially problematic patient, borderline with BDD, and possibly
avoid treating them. This exercise is not always easy since many skillful dissimulators may fool even the most expert
practitioners,but all efforts should be implemented to spot even the most subtle signs of internal psychological pressure during
the initial interview. In rare cases cosmetic and aesthetic perception of personal body appearance by psychologically fragile
patients can even cause problems after simple diagnostic procedures such as a shave biopsy performed to rule out a possible
non-melanoma skin cancer. "Wounding" the unstable perceived aesthetic equilibrium of such patients can generate a
disproportionally aggressive reaction even if perfect healing processes have been assisted by proper explanatory instructions
and wound care. Personal professional charisma and innate emphatic attitude are very valuable weapons to defend ourselves
should controversies arise before or after cosmetic and aesthetic procedures.

independently design and create their own assistants based on their needs, flexibly applying GenAI technology to optimize
workflows without altering existing systems.

UNLOCKING THE POTENTIAL OF EXOSOMES GENERATED FROM INDUCED PLURIPOTENT STEM
CELL-DERIVED SKIN ORGANOIDS FOR WOUND HEALING
The frequency of chronic wounds is on the rise due to the growing prevalence of diabetes, vascular disease, and obesity. This
has resulted in cutaneous wound complications that contribute to clinical morbidity and mortality, impose an economic burden,
and cause emotional distress. stem cell-based therapies have be recognized as potential treatment options. Our previous
work implied a functional role for ABCG2 in the expansion and differentiation of p63-expressing epithelial stem/progenitor
cells. And ABCG2 deficiency in skin impairs re-epithelialization in cutaneous wound healing. We further showed
transplantation of epithelial stem cells can improve healing of chronic wounds. Recent studies also demonstrated the potential
of keratinocyte-derived exosomes to be applied on treating skin disorders. However, the inefficient and limited production of
exosomes has hampered their development for clinical translation. Here, we described an easy and optimized protocol to
produce functional keratinocyte-derived exosomes from human induced pluripotent stem cells (iPSCs). Self-organized
organoids generated from human iPSCs resemble complex skin tissues consisting of epidermis, dermis and pigmented hair
follicles. After 10 weeks of differentiation, the CK14+ cells were seen in the basal layer, whereas CK10 staining confirmed
formation of supra-basal layers. E-Cadherin+ epithelium were also detected. By day 120, CK17 staining indicates
development of outer root sheath of the hair follicles. The stimulatory effects on proliferation and migration of human dermal
fibroblast were observed after applying conditioned medium of iPSC-derived skin organoids to fibroblast culture. The
exosomes were further isolated from concentrated conditioned medium of iPSC-derived skin organoids followed by size
exclusion chromatography. These purified exosomes assessed by NTA and characterized by TEM were positive form CD9,
CD63 and CD81. We validated exosomes isolated from iPSC-derived skin organoids exerted similar effects in wound healing
assay. Further analysis of these purified exosome revealed enrichment of signaling regulator associated with
keratinocyte-fibroblast communication and signal transduction The findings of the current study reinforce the potential of
exosomes derived from iPSC-derived skin organoids can be used to improve wound healing.

patient-centered factors, including quality of life and overall disease burden. Current consensus supports treatment strategies
that are adaptable—not only based on acne severity but also on psychosocial impact and long-term sequelae. The
Personalised Acne Treatment Tool (PATT) highlights important considerations such as truncal involvement and the presence
of acne sequelae, including scarring, post-inflammatory hyperpigmentation (PIH), and post-inflammatory erythema (PIE).
Retinoids remain central to acne management due to their comedolytic, anti-inflammatory, and anti-pigmentary properties.
This talk will explore their critical role in addressing both active acne and sequelae, with a focus on skin of color, particularly

Acne vulgaris is a multifactorial inflammatory skin disorder involving follicular hyperkeratinization, excessive sebum
production, microbial dysbiosis, and aberrant immune responses. While conventional treatments—such as topical retinoids,
benzoyl peroxide, antibiotics, and hormonal therapy—have remained largely unchanged for decades, recent advances have
introduced novel therapeutic agents. Clascoterone, a topical androgen receptor inhibitor, and trifarotene, a selective retinoic
acid receptor-γ agonist, represent significant additions to the acne treatment armamentarium, offering improved
efficacy and tolerability. Concurrently, growing awareness of Cutibacterium acnes antibiotic resistance and
treatment-associated dysbiosis has shifted research focus toward microbiota-preserving strategies. Investigational therapies
targeting microbial biofilms and promoting a balanced skin microbiome are currently under development, aiming to reduce
inflammation while minimizing disruption to commensal flora. These microbiome-centered approaches are also being
translated into next-generation cosmeceuticals, reflecting the evolving paradigm in acne management. This presentation will
provide an overview of recently approved treatments and highlight emerging therapeutic strategies that align with precision
medicine and skin microbiome preservation.

LIPOLYTIC INJECTIONS FOR FACIAL CONTOURING IN KOREA: COCKTAIL FORMULATIONS, INJECTION
TECHNIQUES AND COMBINATION TREATMENT WITH BOTULINUM TOXIN
In Korea, Lipobin (phosphatidylcholine/deoxycholate), previously known as PPC injection, was widely used for fat reduction
(lipolysis), but had its approval revoked in 2014. Since then, several deoxycholate products have been introduced to the
market. However, lipolytic injections (fat-dissolving injections), used as off-label cocktail therapies, continue to be widely
practiced in the field in Korea. This lecture will focus on these lipolytic injections, commonly referred to as "fat-dissolving
injections' or "contour injections,' rather than the deoxycholate products that have been frequently discussed in recent
presentations.
Facial fat changes with aging. While deep fat compartments tend to lose volume, superficial fat shifts downward due to
gravity, leading to increased volume and sagging in the lower compartments. Common areas where volume increases and
sagging occur include the jowl fat, nasolabial fat and submental fat.
While various treatments, such as energy-based devices (EBD) and thread lifting, are used to "lift' sagging fat, targeting fat
reduction itself may be more effective in some cases. Moreover, combining lipolytic injections with other treatments often
leads to higher patient satisfaction.
Lipolytic injections are referred to by different names depending on the clinic, and the agents and cocktail formulations used
vary as well. This lecture will cover the types of agents that can be used for lipolytic injections in Korea, along with their
indications and treatment techniques, and approaches to combining them with energy-based devices (EBD) or botulinum toxin
for enhanced results.

Skin boosters have become a key part of dermatologic treatments, and it is now hard to find an area where they are not used.
In recent years, we have seen a rapid increase in the number and types of skin boosters and biostimulators available. While
biostimulators are effective for skin rejuvenation by helping restore the extracellular matrix (ECM), including collagen and
elastin, they also have some limitations — especially when it comes to delicate areas like the lips or under the eyes.
This is why a multi-layered, multi-approach treatment strategy is essential to achieve optimal skin quality. Combining different

THE COLLAGEN MIRACLE AND WHY IT'S NOT ENOUGH FOR YOUTHFUL REJUVENATION
The Collagen Miracle and why it's not Enough for Youthful Rejuvenation
This presentation is based on a metanalysis of several scientific articles. All traumatic procedures increase collagen. Collagen
is the scar developing over a healing wound. Excessive production of collagen results in keloids. Since open injuries are
hideous. large laser and RF corporations invented the "invisible wound" that is formed under the surface of the skin. They
succeeded in contriving a method of forcing the body to increase its collagen in a certain area, but without investigating
whether the collagen increase at the treatment site was accompanied by deficient amounts of collagen in neighboring areas.
Trauma-based treatments never tested the null hypothesis as they should, according to validity and reliability principles. They
never examined if there is a systemic collagen deficiency in areas other than those receiving the laser or RF treatment. Their
counterargument would probably be: "Let's laser and RF more areas!" Forgetting that collagen increase depends on the
body's supplies and capacity to produce the collagen. If the body was unable to supply this protein lasers and RF would
increase nothing! The glorification of "collagen" has rendered it synonymous with youth maintenance. There are 7 types of
collagen involved in skin integrity and 18,127 proteins determine the age of the human epidermis only. Around 8,987 proteins
control the outer epidermis, and the stratum corneum, and 9,140 proteins are the brains of the inner epidermis, comprising
proteins in the entire body.
Importantly, one cannot have true rejuvenation without proteostasis: the balance of proteins within their optimal range and
their harmonious interactions. Proteostasis is what determines aging and disease. Hormones are proteins and the deleterious
effects of hormonal imbalance have been widely explored and are well known.
Proteostasis is the dynamic regulation of a balanced/functional proteome. Aging is the loss of protein homeostasis. The body
cannot produce collagen because of the inefficient protein-to-protein communications and the systematic unfolding of proteins
that renders proteins non-functional interfering with the instructions necessary for the body repair mechanisms to produce new
collagens. The proteostasis network includes competing and integrated biological pathways within cells that control:
* biogenesis
* protein folding
* protein trafficking
* protein degradation.
A number of cellular processes are necessary for true antiaging. These include metabolic, multicellular organismal processes,
extracellular structure organization, protein development and transport, catabolic events, and adequate cellular
communication and stimulus-response
This presentation is designed to unpack the Collagen miracle to see if the Emperor has no clothes. Hundreds of devices
market themselves as increasing the production of collagen forgetting that keloids are an excess of collagen. Collagen is a
protein like hormones are proteins and it is crucial for anti-aging doctors to understand collagen as one of thousands of other
proteins that need to interact optimally with other proteins in order to turn back the clock. Protein balance is the same as
hormonal balance. Without balance, the freshness and natural glow of youth are forever lost and patients look puffed up and
wrinkle-free, yet artificial. Importantly, they don't look younger because they don't have that freshness and natural glow of
youth, so most people around them can still guess their real age.

ONYCHOMYCOSIS CAUSED BY UNCOMMON FUNGAL PATHOGENS
Dermatophytes are the primary pathogens of onychomycosis. However, there are still many fungi with keratinolytic activity
that can cause nail infections. The prevalence of non-dermatophyte mould onychomycosis (NDM OM) varies by region,
ranging from 1.8% to 23.5% (average 6.9%). The pathogens are highly diverse, mainly Aspergillus, Scopulariopsis, Fusarium,
Neoscytalidlium, etc. Because the pathogens of NDM OM are mostly environmental fungi, their clinical significance should be
carefully evaluated. Correct diagnosis can avoid unnecessary oral drug treatment or missing the true underlying diseases.
Diagnostic criteria include repeated isolation of the same pathogen (species level), identical repeated PCR results, and
evidence of nail invasion by histopathology. Response to treatment varies depending on the pathogen. The treatment duration
is longer than that of dermatophyte OM in general.

contact with sick animals or trauma. Microsporum canis is the most common zoonotic dermatophyte from cats and dogs.
However, along with the popularity of pet keeping and the diversity of exotic species, emerging pathogens are transported
between countries and have gradually appeared in Taiwan and neighboring Asian countries. Fungal culture and identification
are crucial tools for detecting zoonotic infections, playing a vital role in confirming the source of infection and interrupting the

THE STUDY TO EVALUATE THE EFFICACY AND SAFETY OF POLY-D, L-LACTIC ACID HYBRID WITH
NON-CROSSLINKED HYALURONIC ACID INJECTION FOR FACIAL REJUVENATION IN ASIANS
Background/Objectives:
Background: Hybrid PDLLA biostimulator has gained interest in aesthetic field. Little is known about its biophysical effects on
skin and no established clinical study for reconstitution protocol.
Objective: To evaluate the efficacy and safety of Hybrid biostimulator composed of Poly-D, L-Lactic Acid (PDLLA) hybrid with
non-crosslinked hyaluronic acid utilizing novel reconstitution and injection protocol for facial rejuvenation in Asian.
Methods:
Thirty participants, with a mean age of 40.2 ± 7.21 underwent hybrid PDLLA (PDLLA 42.5 mg with HA 7.5 mg) reconstitute
with sterile water 6 cc and lidocaine 1.5 cc to make final of 7.5 cc solution and injecting in subdermal plane utilizing cannula
techniques (0.5 ml infraorbital, 0.25 ml crowfeet and 3 ml midcheek area per side) and hybrid PDLLA (PDLLA 170 mg hybrid
with HA 30 mg) reconstitute with sterile water 7 cc and lidocaine 1 cc to make final of 8 cc dilution and inject to nasolabial fold
1 cc per side. Three treatment sessions spaced 1 month apart were performed with Follow up visits at 2 weeks and 1, 3, 6, 9,
12 months after treatment. Skin biophysical properties were measured at each visit using the Cutometer®, Mexameter®,
Colorimeter®, Sebumeter®, and Antera®
Results:
Currently, patients were follow-up to 6th months after completed 3 sessions treatment, a statistically significant increase in the
skin R0 parameter, measured by Cutometer®, representing abilility of the skin to be streaching upward, was observed starting
from the 2nd treatment and continuing up to 6 months after the complete treatment (p-values = 0.002,
Conclusions:
The hybrid Poly-D,L-lactic acid injections demonstrated improvements in skin pliability and elasticity. Additionally, significant
reductions in depression volume were observed in the tear trough and nasolabial fold areas, along with decreased
pigmentation in the cheek and periorbital regions suggested its potential as an effective treatment option for facial
rejuvenation, reduce scar firmness, diminish pigmentation, as well as providing volumization in depressed areas and minimize
side effects.

Abstract
Background:
Return electrode placement in monopolar radiofrequency (MRF) significantly influences thermal distribution, impedance
stability, and penetration depth. Clear guidelines remain lacking.
Objective:
To identify optimal return electrode placement for enhanced safety and efficacy in MRF treatments through mathematical and
physical analyses.
Methods:
Following a clinical incident involving a first-degree burn, theoretical models including electric field analysis, impedance
variation (IV), impedance variation rate (IVR), and penetration depth were utilized.
Results:
Close electrode placement (<0.2 m) increased localized heating and burn risks, while excessive distance (>0.5 m) decreased
therapeutic efficacy. Optimal placement at approximately 0.45 m balanced effective heating and safety.
Conclusion:
These findings are based on parameters from commonly used systems such as Thermage MRF. If device settings differ, all
calculations related to return electrode placement must be re-evaluated accordingly.
Placing the return electrode around 0.45 m from the treatment area on the back or abdomen provides the best balance of
safety and effectiveness. Further studies should validate these findings clinically.

PERSPECTIVES
Preferentially Expressed Antigen in Melanoma (PRAME), a cancer/testis antigen, plays a significant role in melanoma
pathogenesis and represents a focus in skin cancer research. As a nuclear receptor and transcriptional regulator, PRAME
inhibits retinoic acid signaling—a pathway crucial for cell differentiation and proliferation—thereby contributing to
tumorigenesis, epithelial-to-mesenchymal transition, and immune evasion.
The diagnostic utility stems from the differential expression pattern: PRAME is typically highly expressed in melanoma while
showing weak or absent expression in benign nevi. This distinction enhances diagnostic accuracy in differentiating malignant
melanoma from benign melanocytic lesions. Additionally, PRAME expression correlates with more aggressive melanoma
features, positioning it as a valuable prognostic marker.
The diagnostic applications of PRAME extend to various melanoma subtypes, including uveal melanoma (where expression
correlates with metastasis risk), acral melanomas (improving diagnostic accuracy in histopathologically ambiguous cases),
and potentially in differentiating melanoma from histologically similar entities like clear cell sarcoma or detecting lymph node
metastasis.
Beyond diagnostics, PRAME represents a promising therapeutic target. Its cancer-specific expression pattern makes it an
attractive candidate for immunotherapeutic approaches. Current research explores PRAME-targeted cancer vaccines and
adoptive T-cell therapies. However, challenges such as tumor heterogeneity and immune suppression mechanisms must be
addressed to optimize these therapeutic strategies.
Ongoing research into PRAME's molecular mechanisms continues to advance diagnostic and therapeutic applications,
offering hope for improved management of melanoma—a cancer that, despite recent treatment advances, still requires more

CONSIDERATIONS ON THE MANAGEMENT OF LONGITUDINAL MELANONYCHIA IN CHILDREN
The optimal management of longitudinal melanonychia (LM) in pediatric populations remains clinically challenging. Given the
rarity of nail unit melanoma in children and their unique age-related characteristics, a "wait-and-see policy" has been widely
adopted. This study retrospectively analyzed 74 pediatric cases (≤18-year-olds) with excised LM in our department from
with onset ages of 1, 3, 9, and 10 years (diagnosis confirmed at ages 8-13 years), all located on fingers. In comparison,
among 488 adult LM cases, 75 melanomas were diagnosed, including 4 cases with disease onset ≤18 years (initial
presentation ages: 5, 11, 16, and 18 years; diagnosis confirmed at 25-36 years), all exclusively involving fingernails.
Conclusion: Pediatric nail melanoma, though rare, exhibits early-onset potential. Progressive LM during adolescence warrants
heightened vigilance. Biopsy should be considered for lesions showing progression, particularly during peripubertal stages.

The skin is a complex and extensive organ that harbors both innate and adaptive immune effector cells. These immune cells
serve as a crucial first line of defense against pathogens; however, genetic predisposition and environmental influences can
disrupt immune signaling, potentially resulting in hypersensitivity reactions. Among children, the most prevalent allergic skin
disorders include atopic dermatitis, urticaria/angioedema, and contact dermatitis. Atopic dermatitis (AD) and food allergy (FA)
are prevalent chronic conditions that significantly affect quality of life and contribute to a substantial socioeconomic burden.
AD is closely linked to the development of immunoglobulin E (IgE)-mediated FA. The underlying immune response involves
allergen-specific T helper type 2 (Th2) cells, which promote the release of pro-inflammatory cytokines, such as interleukin
(IL)-4 and IL-13. This cytokine activity drives B cell immunoglobulin class switching, leading to the production of specific IgE
(sIgE). In this lecture, I will share concise overview of pediatric atopic dermatitis, emphasizing recent advancements in their

USE OF ADVANCED SYSTEMIC TREATMENTS FOR PEDIATRIC AD? CURRENT EVIDENCE
Moderate-to-severe atopic dermatitis (AD) affects approximately 20-30% of pediatric patients, often resulting in significant
pruritus, sleep disruption, and impaired quality of life. While topical therapies remain the first-line treatment, a growing subset
of children requires systemic interventions for adequate disease control. Recent advances in immunopathogenesis have led
to the development and regulatory approval of targeted systemic therapies, including biologics and Janus kinase (JAK)
inhibitors, which have transformed the treatment landscape. This presentation will provide an evidence-based summary of the
current efficacy and safety data for biologic agents—namely dupilumab, lebrikizumab, and tralokinumab—and oral JAK
inhibitors including abrocitinib, upadacitinib, and baricitinib in the pediatric population. Dupilumab, an IL-4 receptor a
antagonist, is currently approved for children as young as 6 months and demonstrates robust, sustained efficacy with a
favorable safety profile and no need for laboratory monitoring. Lebrikizumab and tralokinumab, both IL-13 antagonists, show
promising efficacy in adolescents, though response onset may be slower compared to dupilumab. JAK inhibitors offer rapid
symptom relief, with upadacitinib and abrocitinib achieving the highest short-term EASI-75 rates (often >70-80%) and
demonstrating marked pruritus reduction within 1-2 weeks of initiation. Baricitinib, which has been approved for pediatric use
in some regions, also shows meaningful improvement, particularly in children aged 2-11 years, although its efficacy appears
more modest relative to JAK1-selective agents. While JAK inhibitors are generally well tolerated in the short term, they require
ongoing laboratory surveillance due to class-wide warnings regarding infections, thrombosis, and laboratory abnormalities.
Across trials, adverse events such as conjunctivitis (with biologics) and mild acne or gastrointestinal upset (with JAK
inhibitors) were generally manageable and infrequently led to treatment discontinuation. Ethnicity-based subgroup analyses
suggest consistent efficacy across Asian and Western pediatric populations. The selection of an optimal systemic therapy
must balance efficacy, onset of action, safety, administration route (subcutaneous vs. oral), age-specific approvals, and
access considerations. Shared decision-making with caregivers is essential, particularly when discussing potential long-term
safety data and real-world evidence that continue to emerge. In conclusion, biologics and JAK inhibitors represent a
significant advancement in pediatric AD management, offering rapid, targeted, and sustained disease control with the
potential to significantly enhance quality of life for affected children and their families.

Recent advances in noninvasive modalities for assessing cutaneous microcirculation, including nailfold capillaroscopy, laser
Doppler flowmetry (LDF), optical coherence tomography, hyperspectral imaging, and thermography, have broadened clinical
applications in both dermatologic and systemic disorders. These techniques are employed in a wide range of conditions, from
skin diseases (e.g., psoriasis, eczema, vitiligo) to systemic disorders (e.g., diabetes, systemic sclerosis, lupus), enabling early
detection of microvascular dysfunction, monitoring of disease progression, and prediction of therapeutic responses. Our
findings demonstrate that LDF combined with cold stress testing reveals subclinical microangiopathy in systemic sclerosis,
while pretreatment LDF patterns predict response to visible light therapy in vitiligo. Such microcirculatory biomarkers hold
promises for enhancing early diagnosis and guiding personalized treatment strategies, ultimately and significantly improving
overall clinical outcomes.

TREATMENT OF ATOPIC DERMATITIS
Lebrikizumab, an IL-13 targeting monoclonal antibody, has demonstrated robust efficacy in treating moderate-to-severe atopic
dermatitis (AD). In the ADvocate1 and ADvocate2 trials, significant improvements in EASI were observed at 16 weeks, with
many patients achieving EASI75 and EASI90 responses. Responses were maintained over 52 weeks with every 4-week
dosing. The potential for disease modification is also suggested by withdrawal data, as improvements were maintained in
about 50% of patients 36 weeks following drug discontinuation. The ADjoin study confirmed long-term efficacy, with sustained
skin clearance for up to 3 years and minimal use of rescue therapies. Conjunctivitis rates with both short-term and long-term
are less than 10%. In the ADapt trial, lebrikizumab provided clinically meaningful improvements in skin disease, itch, sleep
interference, and skin pain in patients previously treated with dupilumab. Lebrikizumab's high binding affinity and low
dissociation rate likely contribute to its effectiveness. In conclusion, lebrikizumab offers a promising new treatment option with
sustained efficacy, minimal need for additional therapies, and an acceptable safety profile for patients with moderate-severe
AD.

REEPOT®: A NEW TREATMENT OPTION FOR EPIDERMAL PIGMENTATION REJUVENATION WITH
VASCULATURE SALVAGE LASER SURGERY(VSLS)
What is Reepot®?
The concepts and theories introduced above have been concretized into a procedure named "Vasculature Salvage Laser
Surgery (VSLS)," with the first laser system developed based on this concept being the Reepot® laser. The Reepot® laser,
based on a Q-switched 532nm wavelength, is the world's first targeted laser capable of actively controlling hemoglobin
chromophores associated with side effects and reduced treatment efficacy. This control is achieved through an advanced
cooling system and lesion recognition algorithm. Additionally, the laser system has been significantly improved in terms of
user convenience and safety by incorporating a new handpiece design and AR glasses.
1. Why Use 532nm?
The absorption coefficient graph shows that the 532nm wavelength has high selectivity for both melanin and vascular targets.
Historically, Q-switched 532nm was less favored for epidermal pigmentation due to its tendency to damage blood vessels,
leading to a preference for lasers like the 755nm Alexandrite or 1064nm Nd, which have lower selectivity for blood vessels.
However, if cooling can exclude blood vessels, the 532nm wavelength, which has 2.5 to 3 times higher selectivity for melanin,
can remove epidermal pigment more effectively.
2. Why Reepot ®'s Cooling is Different
Conventional contact cooling lasers indirectly transmit cold to the laser-treated area, as direct cooling of the laser glass
causes frost to form inside the handpiece, obstructing the laser beam. This indirect cooling makes it difficult to maintain a low
temperature and causes the temperature to rise sharply when the laser is in contact with the skin. Reepot ®'s technology
cools the entire glass without frost formation (CPTLTM), maintaining a consistent temperature even in contact with the skin,
thus sufficiently cooling the lesion.
3. The Importance of Lesion Recognition Algorithms
Solar lentigines, once thought to be simple pigment disorders, are now known to result from keratinocyte malfunction. Solar
lentigines develop due to increased proliferation and decreased turnover of malfunctioning keratinocytes, leading to melanin
deposition and brown discoloration. The variation in the intensity of solar lentigo colors seen in clinical practice is likely due to
differences in melanin content. Therefore, treating solar lentigines requires complete removal of mutated keratinocytes to
reduce the possibility of recurrence, rather than merely decreasing melanin content. Reepot ®'s fixed 4mm beam with 50%
overlap ensures uniform laser coverage of the lesion, making it far more consistent than manual handpiece operation.
Conclusion
The newly launched Reepot ® laser is expected to minimize side effects while significantly reducing the number of treatment
sessions required for common epidermal pigmentation issues like solar lentigines. Additionally, it offers a new treatment
option for more challenging pigment disorders such as Becker's melanosis, PUL, and café-au-lait spots, which traditionally
required over 20 treatment sessions

REAL-WORLD EXPERIENCE WITH ULTRACLEAR® LASER IN ASIAN SKIN: BALANCING EFFICACY AND
SAFETY IN SKIN REJUVENATION
Background:
Asian skin, typically classified as Fitzpatrick skin types III to V, poses unique challenges in energy-based treatments due to
increased melanocyte activity and a higher risk of post-inflammatory hyperpigmentation (PIH). UltraClear®, a novel 2910 nm
cold-fiber fractional laser, offers a promising solution for skin rejuvenation with minimal thermal damage.
Objective:
To share real-world clinical experience using UltraClear® laser in Asian patients, focusing on treatment outcomes, safety
profile, and patient satisfaction.
Methods:
A retrospective case series was conducted in aesthetic dermatology clinics involving Asian patients undergoing UltraClear®
treatments for indications including photoaging, acne scarring, skin texture irregularities, and pore refinement. Parameters
were adjusted according to skin type and clinical goals. Post-treatment care protocols were standardized to mitigate potential
complications.
Results:
Across asian patients, the majority demonstrated significant clinical improvement in skin clarity, texture, and radiance within 1
to 3 sessions. Downtime was minimal (1-3 days of erythema and desquamation), and no major adverse events were reported.
The incidence of PIH was extremely low when appropriate energy settings and post-care protocols were followed.
Conclusion:
UltraClear® laser provides an effective and safe option for skin rejuvenation in Asian populations when personalized protocols
are applied. Its unique cold-fiber fractional technology minimizes thermal damage, making it suitable for melanin-rich skin
types. Further studies with larger cohorts and long-term follow-up are warranted to reinforce these findings.

This study presents PLLA-SCATM regenerative gene upregulation based on 3 month biopsy analysis examining inflammatory
and regenerative responses to the product. Aside from controlled inflammation, this analysis also demonstrates that
PLLA-SCATM upregulated multiple adipogenic genes, which may suggest regenerative involvement of all three skin layers
(epidermis, dermis, and subcutaneous fat).
